Keto flu: why it happens and how to end it in 48 hours

Dr Sergey Saadi

“Keto flu” is a classic beginner problem: on days 2–5 you get headache, fatigue, irritability, muscle weakness and disturbed sleep. It is NOT flu and NOT a reaction to fat – it is electrolyte depletion, and it resolves within 24–48 hours with the right protocol.

What is actually happening

As insulin drops in the first days of keto, kidneys stop holding sodium and dump water – 3–4 g of water per gram of glycogen. Two kilos of fluid can go in 48 hours, taking sodium, potassium and magnesium with it (Bostock et al., Front Nutr 2020). Headache and cramps are electrolyte loss, not ketosis.

What the literature suggests watching

  • Sodium – losses peak in the first days; needs are individual
  • Potassium – from food: avocado, spinach, salmon, pumpkin seeds
  • Magnesium – well-absorbed forms (glycinate, malate)
  • Fluid – by thirst; excess worsens sodium loss
  • Broth – a food source covering all three minerals
  • Amounts depend on health and medication – with kidney, heart or blood-pressure conditions a doctor decides them
